چکیده

Biological drugs that have a protein or peptide structure provide new and highly eff e c t i v e therapies for a number of conditions and diseases, but their prolonged use in patients gives rise to new problems and questions, amongst them the possible occurrence of neutralizing antibodies (Nabs). We discuss some aspects of this particular problem in relation to interferon-beta therapy of multiple sclerosis (MS). Interferon beta-1b is the first effective longterm therapy for MS. It has been shown to reduce in a significant way the number and severity of MS attacks both in relapsing-remitting and in secondary-progressive MS and to postpone motor and other forms of disability – as expressed in the extended disability status score (EDSS) – as well as to delay, in the more advanced progressive form of the disease, the point at which a patient becomes wheelchair-bound. It also has a strong impact on the “burden of disease” and on the occurrence of new CNS lesions, as evidenced by MRI, which indicates an impressive suppression of the acute inflammatory process in MS. Interferon beta is a human cytokine with non-specific anti-viral and anti-proliferative effects; it is also

Human Leukocyte Antigen Genes and Interferon Beta Preparations Influence Risk of Developing Neutralizing Anti-Drug Antibodies in Multiple Sclerosis

A significant proportion of patients with multiple sclerosis who receive interferon beta (IFNβ) therapy develop neutralizing antibodies (NAbs) that reduce drug efficacy. To investigate if HLA class I and II alleles are associated with development of NAbs against IFNβ we analyzed whether NAb status and development of NAb titers high enough to be biologically relevant (>150 tenfold reduction unit...

Neutralizing antibodies to multiple sclerosis treatments.

OBJECTIVE This article reviews the incidence and clinical significance of neutralizing antibodies (NAbs) in patients with multiple sclerosis (MS) undergoing treatment with interferon beta (IFNbeta). Implications for practice are also discussed in light of the currently available data on the clinical consequences of NAbs in patients with MS. SUMMARY As with other recombinant protein drugs used...

Neutralizing antibodies are associated with a reduction of interferon-β efficacy during the treatment of Japanese multiple sclerosis patients.

Multiple sclerosis (MS) is a chronic immune-mediated inflammatory demyelinating disease of the central nervous system. Interferon-β (IFN-β) has been used as the first line therapy for MS treatment in Japan, but patients treated with IFN-β may develop antibodies, known as neutralizing antibodies (NAbs), which abrogate its therapeutic effects. Intramuscular IFN-β 1a and subcutaneous IFN-β 1b ar...

Neutralizing antibodies explain the poor clinical response to Interferon beta in a small proportion of patients with Multiple Sclerosis: a retrospective study

BACKGROUND Neutralizing antibodies (NAbs) against Interferon beta (IFNbeta) are reported to be associated with poor clinical response to therapy in multiple sclerosis (MS) patients. We aimed to quantify the contribution of NAbs to the sub-optimal response of IFNbeta treatment. METHODS We studied the prevalence of NAbs in MS patients grouped according to their clinical response to IFNbeta duri...

Guidelines on use of anti-IFN-B antibody measurements in multiple sclerosis: report of an EFNS Task Force on IFN-B antibodies in multiple sclerosis.

Therapy-induced binding and neutralizing antibodies is a major problem in interferon (IFN)-beta treatment of multiple sclerosis. The objective of this study was to provide guidelines outlining the methods and clinical use of the measurements of binding and neutralizing antibodies. Systematic search of the Medline database for available publications on binding and neutralizing antibodies was und...
